Why am I asking?
Each type of review requires different information. Choosing the right review makes sure you receive the correct preparation form after scheduling so I can have the information I need before our appointment.

Medicare Advantage Review

Choose this if your Medicare Advantage plan is changing or ending, you want to review or compare Medicare Advantage plans, or you are considering moving from a Medicare Supplement to Medicare Advantage.

Schedule Advantage Review

Part D Prescription Drug Review

Choose this if you plan to keep Original Medicare or your Medicare Supplement and only want to review your standalone Part D prescription drug plan.

After you schedule: You will be asked to complete a short Medicare information form. Please provide your complete current information, not just information that has changed. Having your current prescriptions, pharmacy and, when applicable, doctors and providers ahead of time allows me to prepare for our conversation.
